A page is a list of components in zones, and a design is a set of shapes for drawing them. Changing the design changes how the blocks are drawn, not which blocks are there.

What can change is the choice of SHAPE: a design that does not offer the particular shape you picked draws its own instead, and any setting the new design cannot use stops being offered in the composer rather than sitting there doing nothing.
